FORMS & INSTRUCTIONS

The following are needed annually for all Marching Band Members new or returning.

• STEP 1: Sign-Up for Marching Band with the Band Director during the Spring. You may have already completed this step! If you already receive our weekly emails, you are signed up and in our database!

  • Sign Up for Marching Band (Skip this step, if you already receive band emails)

• STEP 2: Register with the Athletic Department via FAMILY ID by July 18th.

    • Tutorial Video for New Family ID Users (this is an older video but the meat of the content is still correct).

• STEP 3: Turn in Your Sports Physical Form to Morris Knolls School Nurse by July 17th

  • Sports Physical Form (Read the Below!)

  • Fill out pages 1-2 of the packet in preparation for your sports physical. Your doctor will use page 3 to complete their physical.

  • If your child already had a physical this year (or won’t need another one until after the season starts), simply call or visit your doctor's office to request sign-off using your last physical.

  • If your child is already registered to play another sport and has submitted medically cleared within the last 365 days, fill out page 4, the health update form. An additional physical and paperwork are not needed.

  • The doctor will review your answers at your appointment, fill out and sign page 5, and provide it to you for delivery to the Morris Knolls school nurse.
    (Only page 5 with a physician's signature is required for the nurse)

  • Include the following forms with Page 5 as needed:

    • Athletes with Disabilities Form

    • Emergency Epi-Pen Form

    • Asthma Treatment Plan

    • Diabetes Form

    • Seizure Action Plan

• STEP 4: Submit Your $350 Fair Share Contribution by Aug 09
